Extrapulmonary small cell carcinoma (SCC) is not a common disease, furthermore ureteral SCC is very rare. Ureteral SCC is fatal, but there are no standard guidelines for ureteral SCC due to the rareness of this disease and a lack of experience of treating it. Herein, we report a case of ureteral SCC. A 77-year-old woman was presented with a right flank pain, and a right nephronureterectomy with bladder cuff excision was performed. A polypoid and whitish tumor which measured 2cm, was detected in the distal ureter. Microscopically, the SCC components had high nuclear/cytoplasmic ratio, scant delicate cytoplasm, nuclear molding, and hyperchromatic chromatin. The immunohistochemical staining was positive for synaptophysin, chromogranin, and neuron specific enolase. Consequently, we diagnosed the patient as having primary ureteral SCC. In abdominopelvis computed tomography of 6 months after surgery, findings of recurrence were detected. However until 9 months after operation she was alive.
